Snapshot Questionnaire

Before we begin, please enter your unique ID (uid) if you were provided one, this allows us to link your responses to your order. A new UID will be assigned to you otherwise.
We will send your results in bar chart form to your e-mail once we run your responses through our system! We manually run each response locally and write a short interpretation of your results to help make sense of the bar chart, so it may take up to 24 hours to receive your results via email. If you have not received your results, please email us, sometimes it gets caught in spam or email typos occur!
By completing the snapshot questionnaire, you agree for us to store your responses and use them to calculate your visual descriptor results.

During the questionnaire, be as specific as you'd like. Answers range from 0.0 (0%) (hard disagree) to 1.00 (100%) (fully agreed), adjustable in increments of 0.01 (1%).
If you're unsure or feel something doesn't apply to you, choose 0.5 (50%). Keep in mind there are no wrong answers!
